Add order screen

This screen allows to create an order. Some of the fields shown, are actually not in the order, but in the related flight legs. Thats why after the the order has been created, the edit order screen looks different.

It is possbile to show aiport slot fields as well in this screen. This is controlled by the setting AirportSlotInCreateOrder in Administration.
